Change of Address Packet
Whenever you have an address change, this is the packet, consisting of 4 pages, which is required to be completed for each student.
This packet informs the district of the address change to update the
student's electronic record and notifies the transportation department
of a change in pick-up and drop-off location.
If you move any time during the school year, these are the required forms you will need to complete for each child. Additionally, you will need to complete a new emergency card for the appropriate building level (elementary, middle, high). Please return the completed packet(s) to Mrs. Rachel Archibee, Central Registration, Camden Central Schools District Office, 51 Third Street, Camden, NY 13316.
